Some big changes with eligibility, though it is unclear when all these changes will occur. Here’s what seems to be happening (all hospital links are below this).
Thorncliffe Park residents age 40+ (born 1981 or earlier), with proof of residence, are eligible for vaccination: No appointment is necessary. Dates of these clinics:

Saturday, April 10: 9:30-5pm @ 2 Thorncliffe Park Dr, Unit 5-15
Age 50+ in high-risk neighbourhoods (ID'd by postal codes-see here or footnote1) will be able to book online through the provincial portal on Friday, April 7, for vaccine appointments: Provincial Portal
Vaccine eligibility is being expanded to age 18+ living in at-risk neighbourhoods, in Toronto & Peel. These will likely not be booked through the provincial portal; there will be community outreach/mobile units. Teachers teaching at schools in these areas will also be included. Hot spot postal codes are in the footnotes.
Teachers of special education classes are eligible for vaccination, though it’s unclear when/how this will rollout.
CAMH has added individuals with specific health conditions (diagnosis of mental illness; diagnosis of substance use disorder; dementia). NO geographic restrictions. Booking link: CAMH
Scarborough Health Network is still the only place I can see that is vaccinating individuals in the ‘at risk’ category2. Important to note that this includes pregnancy and diabetes & mental illness. You must be a Scarborough resident or see a physician in Scarborough to qualify. Also added: those age 50+ at postal codes, M1B, M1G, M1J, M1K, M1P, M1T and M1V. Booking: Scarborough Health Network

The City of Toronto is booking residents aged 60+ or born in 1961 or earlier. Starting Friday, April 9 age 50+ in high-risk neighbourhoods (ID'd by postal codes-see here) will be able to book online.
UHN is pre-registrating patients with certain conditions (a long list that includes those with asthma and hypertension). Note that you must live in the UHN catchment area to qualify or receive care at SickKids, Sinai, Toronto Grace, UHN or Women’s College. It seems to me that children are eligible, if they have one of the listed conditions. Also eligible: Age 50+ with postal code starting with M5V, M6E, M6H

North York Hospital
Adults with highest risk & high risk chronic conditions (see below) as well as one essential caregiver/spouse who supports them with personal care and/or activities of daily living.

People who are 50 years of age and older (born in 1971 or earlier) and who live in one of the following postal codes starting with: M2J, M2M, M2R, M3A, M3C, M3H, M4A

Those 50 years of age and older this year (born in 1971 or earlier) who live in postal codes: Malvern (M1B); Woburn (M1G); Eglinton East (M1J); Kennedy Park (M1K); Ionview, Dorset Park (M1P); Tam O'Shanter-Sullivan (M1T); and Milliken (M1V)
Those 50+ with highest risk, high risk and at risk conditions. The at risk category is quite big and includes those with asthma/respiratory disease, hypertension, heart disease, etc (see below). Note that their website says you must be a Scarborough resident or see a physician in Scarborough to qualify.

At-risk: immune deficiencies, autoimmune disorders, stroke, cerebrovascular disease, dementia, diabetes, liver disease, all cancers, spleen problems, respiratory disease, heart disease, hypertension mental illness disorder, substance use disorder, thalassemia, pregnancy, immunocompromising health conditions, or any other disability requiring community support
Highest risk: Organ transplant recipients, hematopoietic stem cell transplant recipients, people with neurological diseases in which respiratory function may be compromised (for example, motor neuron disease, myasthenia gravis, multiple sclerosis), haematological malignancy diagnosed within the last year, kidney disease with estimated glomerular filtration rate (eGFR) under 30
High risk: Obesity (BMI over 40), other treatments causing immunosuppression (for example, chemotherapy, immunity-weakening medications), intellectual or developmental disabilities (for example, Down Syndrome)
